Access Key Release Form

Dear Parent/Guardian,

Each student is allocated a unique access key which is printed on their envelope. A list of all student access keys are also provided to the School administration office.

If the School cannot locate your child's access key, please complete the form below. For Child Safety and Security reasons, we cannot release Access Keys over the phone.

​

Once we receive your access key request, we will confirm your guardianship of the student with the school before emailing your child's access key to you.

Please note: By submitting this form you are making a Declaration of Consent that you are the legal parent/guardian of the student named in the form, and have the rights to obtain photographs from Fotoworks School Photography.
